Healthcare United is seeking a dynamic physician leader to serve as  Nephrology Medical Director (NMD) . This role will oversee the population health management of patients with CKD, working to improve outcomes, reduce total cost of care, and provide value-based, patient-centered care.

The position offers 50% remote flexibility with travel and coverage across multiple Flagstaff locations. Physicians can select from  several practice locations throughout Ohio .

The Nephrology Medical Director will provide clinical leadership and guidance for nephrology services, developing care pathways, overseeing utilization management, and fostering strong relationships with physicians, health systems, and payers. This role includes both  strategic leadership and direct patient care (via telehealth and in-person settings).

• Provide clinical leadership and expertise in nephrology within a value-based care framework
  
• Collaborate with nephrologists, PCPs, hospital systems, payers, dialysis providers, and other stakeholders
  
• Develop and implement evidence-based protocols, clinical pathways, and performance measures
  
• Lead transitional care efforts and strengthen communication across the healthcare ecosystem
  
• Oversee utilization management, quality outcomes, and financial performance for attributed members
  
• Build and manage a nephrology specialty network that supports integrated, high-value care
  
• Educate providers and ensure compliance with regulatory and clinical standards
  
• Provide direct patient care via telehealth to CKD patients as needed
  
• Mentor teams, encouraging collaboration and innovation
  

• MD/DO with Board Certification in Nephrology
  
• Active Ohio medical license (or ability to obtain)
  
• 5–10 years of nephrology clinical experience preferred
  
• BLS certification required
  
• Experience with population health management and value-based care models
  
• Strong leadership, communication, and interpersonal skills
  
• Familiarity with Medicaid, Medicare Advantage, and dual-eligible populations
  
• Knowledge of clinical guidelines and performance metrics in nephrology
