Overview: The Patient Care Coordinator serves as the primary point of contact for patients receiving behavioral health services in an outpatient setting. This role ensures a seamless, compassionate, and efficient care experience by coordinating patient intake, scheduling, insurance verification, treatment planning support, and follow-up care. Working closely with clinicians, psychiatrists, and administrative staff, the Patient Care Coordinator helps facilitate communication between patients and the care team, maintains accurate clinical and administrative records, and supports the overall delivery of high-quality, patient-centered behavioral health care.
Role Requirements: - Bachelor's Degree in psychology, social work, counseling education, mental health counseling, or human services field; or an Associate's and two years of direct human services work experience; or four years of direct human services work experience.
- Must have knowledge of available resources in the service area for adults with serious mental illness.
- Must have knowledge of and comply with state and federal statutes, rules and policies that effect the target population.
- Must be able to provide individualized, integrated mental health, substance abuse and co- occurring services to meet the needs of individuals served and their families.
- Must be able to use a computer.
- Good oral and written communication skills.
- Must be able to pass organization screening requirements, including state or federal background screenings as appropriate.
