PROVIDES SUPERVISION TO: No Supervisory Duties

GENERAL EXPECTATIONS: The Community Housing Nurse plays a vital role in ensuring the health and well-being of residents within the agency’s community housing programs. This position is responsible for monitoring medication administration, maintaining accurate medical records, training staff on proper procedures, and ensuring compliance with healthcare regulations. The nurse will work closely with direct care staff, case managers, and medical providers to coordinate residents' healthcare needs. This position requires availability 25 hours per week, with some evening and weekend on-call responsibilities. All responsibilities must be carried out in accordance with CWTC’s mission and policies, fostering a supportive and inclusive environment for individuals with disabilities. Employees in this role are expected to attend all required training sessions and demonstrate professionalism, accuracy, and compassion in their daily work.

ESSENTIAL DUTIES:

  1. Complete the one-day online IDHS RN Nurse Trainer Course if not already certified
  2. Train Direct Care Staff and Case Managers on Medication Administration according to Rule 116
  3. Assist in the completion of initial move-in assessments as well as annual assessments of residents
  4. Complete progress/case/nursing notes after resident contact
  5. Review and approve medication changes and provide necessary medication information to direct care staff.
  6. Assist in keeping resident’s medical files up to date
  7. Oversee medication errors and implement corrective action in coordination with the Safety Director
  8. Review and sign off on all residents’ annual program plans
  9. Maintain accurate records in the electronic MAR database as well as in resident medical charts
  10. Develop a relationship with the agency’s contracted pharmacy to utilize electronic MAR to provide medications in the safest manner possible

OTHER DUTIES:

  1. Coordinate appropriate staff and resident training with House Managers
  2. Perform other tasks, not inconsistent with qualifications or regular duties as assigned

WORKING CONDITIONS : Physical: This role may involve extended periods of sitting, requiring good hand coordination, as well as clear communication, hearing, and vision to perform essential tasks. Environmental: Work settings may include residential group homes and/or the main office. Interpersonal: Effective communication and collaboration with adults of diverse abilities are essential to fostering an inclusive and supportive environment.

RELATED CONDITION: All employees are responsible for reporting any information, however acquired, about possible abuse or neglect of consumers to the Safety Director or the Executive Director.

SAFETY AND ACCIDENT PREVENTION: Employees must follow all safety policies and use the necessary protective equipment when applicable. Employees are responsible for reporting safety hazards and/or accidents to their supervisors immediately.

Requirements:

MINIMUM QUALIFICATIONS:

  • Registered Nurse license in good standing with the state of Illinois
  • 1 year of professional nursing clinical experience
  • 21 years of age
  • Valid driver’s license and current auto insurance
  • Strong written and verbal communication skills
  • Availability to take calls in the evenings and some weekends
